What Paperwork is required for participation in a NSYSA Program?Printable

To participate in a NSYSA Program, there may be a requirement for a specific form, document, etc. that you must provide to the NSYSA (we call it Paperwork). Generally, you can provide the necessary documents to the District via the U.S. Mail OR you can provide it electronically via Member: Paperwork.

Keep in mind that the required Paperwork can change from year to year, as many of the requirements are imposed upon the NSYSA by other organizations (e.g. the U.S. Government, Washington State, WSYSA, etc.).

That said, the following Programs have the identified requirements:


Referee

Youth Referees (those under age 18) are required to have on file the following documents:

  • Proof of Birth
    Proof of Birth is usually a birth certificate, but other acceptable alternatives can work - such as a valid drivers license, a baptismal record or a hospital birth certificate.

    Send the Proof of Birth document to the District following the directions below concerning Completed Forms/Documents

  • Parent/School Authorization document - This document must be re-submitted on an annual basis.

    The required Parent/School Authorization form can be generated at Member: Paperwork-Authorization.->
    From this screen, simply enter the Name and Address information for the school you are attending, answer the question/s about other jobs and click on Display.

    Next, print out the document and the Youth Referee must sign in the applicable block as the Minor Employee.
    Next, have your parent complete and sign the Parent Authorization Section
    Next, have a school official complete and sign the School Authorization Section
    NOTE: If you are Home Schooled - annotate the Title block of the School Authorization Section with Home Schooled and then the parent should sign as the School Authority.

    Send the completed Parent/School Authorization form to the District following the directions below concerning Completed Forms/Documents

  • W9
    The required W9 form can be generated at Member: Paperwork
    From this screen, simply click on W9 and then fill out the W9 Form on your screen using the instructions provided.{See W9 Sample below}
    Next, print out the document and sign and date the applicable blocks in Part II.

    Send the completed W9 form to the District following the directions below concerning Completed Forms/Documents

  • SSN on file in Members Profile
    Referee's SSN must be on file in the Members individual Profile.
    Click here to go directly to your Member: Profile to view and update the information contained within your profile Member: Profile

  • WSYSA Risk Management Clearance
    You are required to obtain and maintain a valid, up to date Risk Management Clearance through WSYSA.

    To apply for either an initial or renewal of an existing Risk Management clearance, we recommend that you do so via your Club/Association Bonzi website if they have one. If not, then please follow the instructions provided by WSYSA here->

    You can see the current WSYSA RM Status that District IV has on file for you by going to Member: Risk Management

Adult Referees (those age 18 and above) are required to have on file the following documents:

  • W9
    The required W9 form can be generated at Member: Paperwork
    From this screen, simply click on W9 and then fill out the W9 Form on your screen using the instructions provided. {See W9 Sample in Youth Referee Section above}
    Next, print out the document and sign and date the applicable blocks in Part II.

    Send the completed W9 form to the District following the directions below concerning Completed Forms/Documents

  • SSN on file in Members Profile
    Referee's SSN must be on file in the Members individual Profile.

  • WSYSA Risk Management Clearance
    You are required to obtain and maintain a valid, up to date Risk Management Clearance through WSYSA.

Team Staff Member (Coach, Assistant Coach, Manager, Trainer)

Team Staff Members are required to have on file the following documents:

  • WSYSA Risk Management Clearance
    You are required to obtain and maintain a valid, up to date Risk Management Clearance through WSYSA.

    To apply for either an initial or renewal of an existing Risk Management clearance, we recommend that you do so via your Club/Association Affinity website if they have one. If not, then please follow the instructions provided by WSYSA here->

Completed Forms/Documents

In order to send a completed form/document electronically to the District IV Website, you'll first have to have a digital image of the Paperwork saved in a file as a PDF, JPG, or TIF file (PDF preferred). This will require that you have a working knowledge of how to scan a document and are able to save the result as a file that can be uploaded to NSYSAsoccer.org

Once you have scanned a copy of any of these complete forms/documents noted above (Proof of Age, Parent/School Authorization, W9, etc.) to your PC, the resultant file can be uploaded to NSYSA on the website via Member/Paperwork/Upload.->

If you do not have the capability to send your completed documents/forms electronically to NSYSA, please mail the completed forms/documents to:

Once received by the NSYSA, these completed forms/documents will be converted to electronic format and uploaded to the site. You will then be able to view them via Member: Paperwork and see that the requirement to submit the applicable form/document has been satisfied via Referee: Status
